This permit to construct one water well as set forth above, is granted under provisions of Article 10 of the
Putnam County Sanitary Code and Subpart 5 -2 of Part 5 of the New York State Sanitary Code and provided
that within thirty (30) days of the completion of water well construction, the applicant or their designated
representative shall: 1) Pump the well until the water is clear. 2) Disinfect the well in accordance with the
requirements of the Putnam County Health Department. 3) Submit a Well Completion Report on a form
provided by the Putnam County Health Department. During all well drilling operations, the applicant and/or
well driller shall take appropriate action to assure that any and all water and waste products from such
well drilling operations be contained on this property and in such a manner as not t degrade or otherwise

APPROVED FOR CORD TION: This approvxpires two years from the date issued unless
construction of the well has been completed and inspected by the PCHD and is revocable for cause or may be
amended or modified when considered necessary by the Public Health Director. Any revision or alteration

The application to drill a new well at the above referenced lot was approved at this
Department's November 12, 2002 waiver meeting with the following stipulations:
1. Submission of Certificate. of Occupancy for above noted dwelling.
2. A minimum of 53 feet of casing must be provided for well protection.
3. The proposed well location must be staked by a licensed New York State land
surveyor; the location must be confirmed by a representative of the Putnam
County Health Department prior to, the well construction.
As -built plan, Well Completion Report (WC -97), Well abandonment, if applicable, and
water quality analysis shall be submitted no later than 30 days after the well completion
by the permittee.

I have been advised by my constituent, Helen Forcina, who resides at 87 Hewitt
Street in Putnam Valley (Lake Peekskill), that she has had a well permit pending
with the Board of Health for some time. Apparently there is some discrepancy
between her engineer's. distances and the Board of Health's assessment of the
distance required to issue a well permit. Ms. Forcina lives on a fixed income. Her
home is currently served by "summer water'. She will not be able to remain in her

financial situation will be greatly strained if she is forced to rent another house
over the winter.
I respectfully request that Director of Health Foley review this case and advise
me of the situation. Any suggestions regarding remedies that may be available to
Ms. Forcina will be greatly appreciated.

for re- consideration of the previous denial to the Waiver Committee for discussion on October 5,
1999. While the committee and myself are truly sympathetic to your mothers current health
condition, it was determined that the situation with respect to the proposed well and surrounding
septic systems has not changed. Therefore, the re- consideration was denied by the Waiver
Committee.

In response to your memo of September 25, 2000 to County Executive Bondi, please be advised
Ms. Forcina has applied for a permit to drill a well-on her property which is.currently -being
served by a summer water supply (Lake Peekskill). The Department received her application and
after discussion with Ms.,.Forcina.and her engineer, rejected_the application .on:the.basis:the:well..:

sanitary system and 88 feet to an adjacent sanitary system (see letters attached).
Ms. Forcina has stated there are wells which do not meet current separation distances, which is
correct, however these wells were drilled prior to our well permit application requirements. In - _ ____ _ . _ _ . _.
addition, Ms. Forcina and/or. her family has known that this dwelling has always been served by
a seasonal water supply.
Issuing a waiver from our requirements would only serve to set a precedent for the remaining
summer property owners to apply for permits which do not meet code requirements. This would
expose a number of homeowners to the potential of drinking water which may not meet
bacteriological standards and could present a risk to public health.
My suggestion to Ms. Forcina would be for her and others impacted to approach the Town,of
Putnam Valley and request the Town address the long awaited sewer /water issue facing the
residents of Lake Peekskill.

In response to your memo of September 25, 2000 to County Executive Bondi, please be advised
Ms. Forcina has applied for a permit to drill a well on her property which is currently being
served by a summer water supply (Lake Peekskill). The Department received her application and
after discussion with Ms. Forcina and her engineer, rejected the application on the basis the well
-.did not. meet. current separation distances of well -to—septic—,of 100.',--�Her well would be 88_' to_her-
` sanitary system and 8ii feet to an adjacent sanitary system (see letters attached).
Ms. Forcina has stated there are wells which do not meet current separation distances, which is
correct, however these wells were drilled prior to our well permit application requirements. In
addition, Ms. Forcina and/or her family has known that this dwelling has always been served by
a seasonal water supply.
Issuing a waiver from our requirements would only serve to set a precedent for the remaining
summer property owners to apply for permits which do not meet code requirements. This would
expose a number of homeowners to the potential of drinking water which may not meet
bacteriological standards and could present a risk to public health.
My suggestion to Ms. Forcina would be for her and others impacted to approach the Town of
Putnam Valley and request the Town address the long awaited sewer /water issue facing the
residents of Lake Peekskill.

For your information the New York State Health Department will be requiring filtration
of this supply to be in compliance with Part 5 of the New York State Sanitary Code.
2. Pursue sharing of an adjoining well water source through legal easements.

As a six time president.of the Lake Peekskill Civicssoc.(LPCA)
it has been my responsibility to have champion the cause of many
homeowners in the Lake Peekskill Community. 14ost notably, I have been
involved in the proposed Water /Sewer project for Lake ?eekskill, since
1972 a total of 2C years. You will not find a more knowledgeable
person on this topic than yours truly. Enclosed are copies of LPCA
Newsletters dealing on this problem.
Rather than review previous correspondence in defense of Mrs.Forcina
I wish to present My position in support of granting her a "TELL PER11,4IT.
That this action would also require reopening and reviewing her
application for a well permit.....so be it.
1. r:'.r. Foley stated in his letter of 9/25/00... "r:rs Forcina and /or
her family ha.s 1;nown that this dwelling has always been served by a
seasonal water supply ". Gould you agree this certainly applies to
better than 90°'0 of all the properties sold in Lake Peekskill?
Can you visualize if such a restriction had been placed on the
earlier settlers, ,-,,ho made this country great? They surely could not
have developed what we have..THE UNITED STATES OF AMERICA.
2. Nrs. Forcina has a strong HARDSHIP case to war•rant'some special
consideration for a "waiver •request" to grant her a well permit.
Her 91 year young mother cannot live in the Lake Peekskill home
because she has no viell T,�iater. Instead, living in the Brooklyn apartment
she is cared for by non - family members. The added expense also has
created an unforeseen financial burden on the daughter's weekly budget,
in paying strangers and �or friends to tend to her r,,others daily needs.

3. Another financial problem arises for hrs. Forcina in renting
another home that has well water. As you know the seasonal water supply

application was rejected because, "the well did not meet current

5. liar. Norman Anderson, the professional well - driller expert, not
only in the area but also as a resident of Lake Peekskill has determined
upon his examination, he could drill a well without any violations. He
is ready and prepared to drill the well. He too awaits. the Dept of Health
to issue Nirs. Forcina a well permit. If anyone person is knowledgeable
in this area, surely Mr. Anderson has the expertise. rased on his
evaluation, it would he most appropriate for the Dept. of Health to
arant itiirs. Forcina the permit and let Mr. Anderson do his job.
6. Finally, let Fe .en F orca.na.. bri:nq her mother ..:tr__,- 1 i ve in-ac ;sortie-
loving atmosphere with her family. The people in 3rooklyn. cannot give
a 91 year woman the personal loving attention that a daughter wili give,
to her mother. Let the Forcina family get their well to supply thertr
everyday water needs throughout the full year.
7. My conclusion. The Dept of Health should give ilrrs. Forcina
the well permit. Such action will not cause anymore problems that
already exist in Lake Peekskill.

PRESIDENT'S MESSAGE
The Lake Peekskill Water and Sewer project has been the
"impossible dream" of Lake Peekskill. Since 1972, for 28 years,
every Town Board has made some effort to appease the homeowners.
The cost factor went from $6,910,000 to $57,000,000. Is
.it any wonder why the project becomes "impossible "? If the
homeowners' monthly share in 1982 would have averaged $438 a
month, are we to accept that in 2000, the average share would
only be between $199 and $286 a month? Added to this is the.
homeowners' hook -up costs. In the earlier years, the average
amount was $1000 per household, while the current average is be-
tween$1700 and $2500. Not clearly spelled out was the location
site of the home to the hook -up line. Are you down in a gully?
Up a rocky slope? Far back from the pipe line? Location sites
could double or triple hook -up costs to the homeowner.
We have been told that low cost homeowners loans are "out
there '.' Yet no information was ever given on how to apply for
said loans.
The latest report in 1999 to past supervisor Anderson from
Insite Engineering was the first time some revelation was
stated why Lake Peekskill cannot have water /sewer.... "This is
clearly an unaffordable level of expenditure in an area in which
the average household income was $42,000..."

first two are out of the question, while the other two were
"doubtful" for the Lake Peekskill community.

concerning the Water /Sewer project. It's your money elected
officials are spending. Let your voice be heard. ARE YOU "FOR"

prayers for my wife's recovery to good health. My wife's ill-
ness prevented my participation in the monthly. LPCA meetings.
1st Vice Pres. Mel Levenson and 2nd Vice Pres. Ted Muniack ably
performed and conductuJthe official LPCA meetings during my
absence.
The entire membership should be very concerned with the new
leadership of the LPCA. Next month, a nominating committee will
be appointed to consider candidates for the next administration.
Several positions will need to be filled. All interested persons

it.has been my responsibility to have champion the cause of many
homeowners in the Lake Peekskill Community. t4ost notably, I have been
involved in the proposed Water /Sewer project for Lake ?eekskill, since
1972 a total of 2E years. You will not find a more knowledgeable
person on this topic than yours truly. Enclosed are copies of LPCA
Newsletters dealing on this problem.
Rather than review previous correspondence in defense of Mirs.Forcina
I wish to present my position in support of granting her a 'i.ELL PERMIT.
That this action would also reauire reopening and reviewing her
application for a well permit ..... so be it.
1. hr. Foley stated in his letter of 926/00... " Mir s Forcina and /or
her._.f.aanily has k-noirn that this dwelling -has - always - been served -by a "
seasonal water supply ".. '.Jould you agree this certainly applies to
better than 900% of all the properties sold in Lake Peekskill?
Can you visualize if such a restriction had been placed on the
earlier settlers, who made this country great? They surely could not
have developed what we have..THE UNITED STATES OF AMERICA.
2. Klrs. Forcina has a strong HARDSHIP case to warrant some special
consideration for a "waiver request" to grant her a well permit.
Her 91 year young mother cannot live in the Lake Peekskill home
because she has no well water. „Instead,.l.iv_ing in the Brooklyn apartment
she is-cared for by non - family members. The added expense. - also-= rhas:,;
c•r'ea't.e.d an unforeseen financial burden on the dauahter's wee'kl.y...' budget•;
in paying strangers - and /or'friends to tend to her mothers daily needs.

3. Another financial problem arises for bars. Forcina in renting
another home that has well water. As you know the seasonal water supply
will he turned off, shut down, for the winter months on or about Nov.15th.

application was rejected because, "the well did not meet current
separation distances of well to septic of 100 feet. She was 88 feet or
a 12 foot shortage ".
She is surrounded with homes having less than her 6 lots, all of

which a well with their sanitary systems.
5. 11-r. Norman Anderson, the professional well - driller expert, not
only in the area but also as a resident of Lake Peekskill has determined
upon his examination, he could drill a well without any violations. He
is ready and prepared to drill the well. He too awaits the Dept of Health
to issue Mrs. Forcina a well permit. If anyone person is knowledgeable
in this area, surely Mr. Anderson has the expertise. Based on his
evaluation, it would be most appropriate for the Dept. of Health to
grant Mrs. Forcina the permit and let Mr. Anderson do his job.
6. Finally, let Helen Forcina bring her mother to live in a home -
loving atmosphere with her family. The people in Brooklyn cannot give
a gl year woman the personal loving attention that a daughter wiligive,
to her mother. Let the Forcina family get their well to supply.therr
everyday water needs throughout the full year.
7. My conclusion. The Dept of Health should give Mrs. Forcina_ _.,.

4. Since 1972, 1 have been involved in the proposed project. On September 15, 1982, the
then official local newspaper, The. Community Current, reported that on September 11, 1982, by as
referendum vote of 257 to '219, homeowners /taxpayers /residents of Lake Peekskill approved the

All the Town Boards of Putnam Valley knew they had an obligation to offer sewerage dis -y
0 posal to their town. Funding was available, provided Lake Peekskill tied into Westchester Coun

aid and 12'/2% from State aid.
The most recent past administration had indicated that four (4) grant programs were avail-

greater than) $42,000 ". Now we know why the project has been the "IMPOSSIBLE DREAM" for

Since we finally have an answer to why is has taken 28 YEARS for all past administrations

Dear Mr. Hedges,
I am writing this letter on behalf of myself and my mother, who will be 88 years old on
November 3, 1998. I am asking for a permit to construct a water well on my property. I
understand I need 100 ft. from an existing septic system, and I am coming up a little short. I
know I am the only one in my surrounding area without a water well. But, I also know I have the
most property (6 lots.)
My house in LakePeekskill is my primary home. I am here from April until November each year.
My taxes are paid on time, in full, and I vote here as well. In the winter months, my mother and I
live with my daughters at: 8834 Ft. Hamilton Pkwy., Brooklyn, NY 11209. The landlord is
selling the house and now this is the only home my mother and I have to come.
I know you don't have time for this, but I must tell you about my history in LakePeekskill. My
father, Mr. Paul Sarkisian, bought the house in 1940. He applied and received a well permit in
1957, or there about. With all the changes we made over the years, we have not been able to
locate the permit. He happened to apply for the permit with our then neighbor,
Mr. George Donnely.
I spent my childhood here and my children spent their childhood here as well. All of them have a
deep love for our home in LakePeekskill. We also had many family reunions here throughout the.
years. As a result, the Forcina's family, my in -laws, my nieces and nephews all look back with
fond- mem-ories.
My father and my husband are both buried in Hillside Cemetery. Our dream was to retire in our
home in LakePeekskill, but now it's my dream; mine and my mother's. Please don't let it die for a
few feet of ground.

Last March my mother who is 89 years young, had eye surgery, which has left her legally
blind. Due to her age and the surgery, she is unable to make the trip back and forth to the
