PERMIT TO CONSTRUCT A WATER WELL
This permit to construct one water well as set forth above is granted under the provisions
of Subpart 5 -2 of Part 5 of the New York State Sanitary Code, and provided that within
thirty (30) days of the completion of water well construction, the applicant shall:
1. Pump the well until the water is clear.
2. Disinfect the well in accordance with the requirements of the Putnam County Health
Department attached to this permit.
3. Submit a Well Completion Report on a form provided by the Putnam County Health Department.
During all well drilling operations, the applicant shall take appropriate action to assure that
any and all water or waste products from such well drilling operations be contained on this
property and in such a manner as not to degrade or otherwise contaminate surface or groundwater.

the above - captioned project has been completed. Comments are offered as follows:
"The construction of this sewage disposal system may be subject to local wetlands
regulations. You should contact local wetlands officials in this regard."
1. the SSDS and expansion area is proposed on slopes greater than 20% in areas.
Current codes states that an SSDS cannot be approved on slopes greater than
20 %.
2. All separation distances are to be measured from the toe of the slope.
Therefore, minimum distances from the house foundation to the edge of the
trench would be 16 feet.

Dear Mr. Moms:
We ` have reviewed your letter of March 21, 1996 relative to the above referenced matter.
Significant revisions have been made to the proposed SSDS plan (copies enclosed) as a result of
our further study. Those revisions are more specifically as follows:
1. The plan and permit now note 200 linear feet of absorption trench for the two bedroom
house; in accordance with current code.
2.- A minimum separation distance of 16 feet is now indicated to be provided between the last
:...... = trench and the retaining wall.
3. A retaining wall detail has been provided.
While these revisions provide a system in accordance with code, they result in a lesser reserve
expansion area. Although the resulting area available for expansion is not equal to what is
nowadays provided, it is comparable to what was called for in the area when it was earlier
developed and is consistent with State Code.

Dear Mr. Morris:
We have reviewed your letter of June 4, 1996 relative to the above referenced matter.
Significant revisions have again been made to the proposed SSDS plan (copies enclosed)
as a result of our further study. The revised plan along with the following responses,
enumerated in accordance with your. letter, are_provided for your further consideration.
1. The system components and expansion area have been re- configured and overlay
existing slopes that average no more than 20 %.
2. The 20 ft house to SSDS separation has been revised to the theoretical toe of
slope, which would lie 16 ft from the trench ends. As such a 36 ft separation
distance from the trench ends to house is now indicated on the plan.

3. With this revised plan we have to date met all the design requirements conveyed
in your communications. Furthermore, it's our considered opinion that the
proposed plan provides the best design feasible for the existing property.
This design faithfully executes the requirements of the State Sanitary Code and is
considerably more capable than many other sewage disposal systems which function
today in the Lake Peekskill community.
Please carefully review this third design. Comment if any significant environmental
health issues remain un- addressed and if they are due solely to the nature of the parcel. If
it is your determination that the parcel is naturally constrained to such a degree that
construction of a sewage disposal system would be prohibited please so indicate in
writing.
Thank you for your time and efforts in working with us towards a suitable resolution of
this matter.
