I have received and reviewed the plans for the proposed addition to the above - mentioned
residence. The proposal for the addition has been approved as per plans bearing the
approval stamp from this Department dated April 22, 2004. The addition is approved

2. The area of the existing sewage disposal system, and its expansion area, must be

jurisdiction of the Town of Putnam Valley.

To Whom It May Concern:
Enclosed is my respectful request for permission to upgrade my home. I have included all
the documents you requested, including your form; the Certificate of Occupancy; a
sketch of our property showing location of well and septic tank (no survey was ever done
on this property — when I spoke with Linda on 3/17 she checked with another individual
there who said a sketch showing distances from the property line would be sufficient); a
sketch of our existing floor plan; two sets of our proposed floor plan; and a money order
for $100.00.
I know that there is limited building allowed in Lake Peekskill at this time. However, we
do not plan to add another bedroom or add significantly to the square footage of our
house. We are living only on our first floor, which is 672 square feet (well below the
average of 1200), and our laundry and much of our plumbing is outside the house. It is
our hope to convert our attic to a bedroom and change the downstairs bedroom to a
laundry room/bathroom.
We are requesting permission based on what we consider a hardship: our house is not
liveable year - round. As an originally summer -only residence, the original owners left
much of the plumbing outdoors and exposed, to the elements. We have had numerous._ _
occasions of pipes freezing and bursting, and we are unable to use our laundry facilities at
all in the winter ._e bought. this house with the.intention:of living in. it year- round_(and
are ri fact doing so): After going 'thiougli our-second- winter with this..current situation we
have come to understand that without some upgrades living in it year -round will not be
reasonable, affordable, comfortable or safe. We don't believe we can wait until the
current moratorium on building is over.
Kindly look over my request. I know I have several more approval steps ahead of me; I
appreciate your time and attention, and look forward to a favorable reply.

My property is 100.64'wide by 512.49deep (copy of tax map attached),
#83.50 -1 -23, + the adjacent lot to the left. Last year I combined these two
lots into one deed so I would receive only one tax bill;the Building Dept.
has not yet updated this map to show these 2 lots as one.)
The well is +/ -10' in from the road and +/- 20' in from the left property line.
The previous owner mentioned that this well was drilled some time in 2000
because the first well had run dry. I do not know the location of the first well.
The septic system is +/- 70' in from the road, +/- 25' from the right property
line. I do not know when it was installed.
The house is approximately 40' in from the road, and +/ -20' from the right
property line.
