FOLEY. R.S. Acting Public Health Director 1995 I have received and reviewed the plans for the proposed addition to the above mentioned residence. The plans have been approved as per plans bearing this Departments stamp and dated April 5, 1995. The survey indicates that sufficient area exists to expand or repair the sewage disposal system, should it become necessary in the future. Therefore, based on the information submitted, the above mentioned addition is approved with the following conditions: 1. The total number of bedrooms must remain at two without prior approval by this Department. 2. The determination of whether a proposed room addition to a house is considered a bedroan will be made by Department staff based upon: -_ • - location of the roan in the house - size of tie roam. _- 2. Any addition which is considered a bedroom requires a formal approval of plans, (Construction Permit) by the Department which plans are to be prepared by a Professional :-Ingineer in accordance with applicable. sect.ions... d?1 Tres,... Any addition which is not a bedroom but which increases the living area by 15% or more; example, enlarging living room, enlarging kitchen, adding a den, requires that SDS ex-- ansion area of at least 50% or.more be documented as available, no closer than 100 feet to existing wells and /or watercourses. A plan may be prepared by the property owner showing house floor plan, before and after, location of water supply source, existing and pfd SSDS area on this lot and adjacent lots, within 200 feet. 1'°1-Dl'`' f An approval letter will be issued by the Department. It will generally not be necessary to install the sewage facilities at the time of the approval unless problems exist or are suspectd to exist with the operation of the - sewage system or if the addition is significantly larger than the 15 %. Such decision to require the installation of additional trenches at this time will be made after consultation with the Director. Any addition of 15% or more; example, enlarging living room, enlarging kitchen, requires SDS expansion of at least 50% or more, no closer than 100 feet to existing wells and or water courses. Separation to your own well shall not be less than presently located. This to be determined by a representative of this ° Department. 3. All additions must include the installation of water saving plumbing facilities. These devices include, but are not limited to, low flow toilets, shower heads, etc.
