This Department has received and reviewed the plans for the proposed addition to the above
mentioned residence. The proposal for the addition has been approved as per plans bearing the
approval stamp from this Department dated July 23, 2014. The addition is approved with .the
following conditions:
1. The total number of bedrooms must remain at three without prior approval by this
D"epa tmeiit....
2. The area of the existing sewage disposal system and its expansion area must be
maintained.
3. All plumbing fixtures must be updated with water saving devices, i.e., new low flush
toilets, restrictors for shower heads and faucets, etc .. .
4. The approval is for the modifications only and does not validate any construction shown
as existing that has not obtained proper approvals from other agencies having
jurisdiction.
5. This approval is valid for two (2) years and expires on July 23, 2016.
Any permits or variances required under the jurisdiction of the.Town of Putnam Valley are the
responsibility of the applicant.

Dear Mr. Reed,
In response to your questions dated July 2nd 2014 we enclose revised floor plans. We have
relabeled the floor levels to avoid any confusion, because the site is steep one enters on the
second floor, which we are now referring to as the entry level as this seems more clear (in
previous set we called it the ground floor but we ea Ize that might be misleading). The
lower level is still referred to as the lower level.
I have enclosed one copy of the existing plan and two copies of the proposed floor plan.
Please note, the scope of work has not changed and we have addressed the concerns you
noted below.
1. Page Al, existing loft shown above entry level, loft to be removed (therefore not shown on
proposed plan).
2. Page Al, staircase from entry level goes down to lower level (our previous note was an
abbreviation for down 'dn' which we realize oddly enough looks exactly like up, but upside
down. we have changed this to read "down ".)
3. See #2 above.
4. See #1 above, the loft is to be removed.
I would appreciate it if you could confirm receipt of the package . and pleaseilte m eknow if
you have any questions.

regarded project has been completed. Comments are offered as follows.
1. Page Al; existing ground floor plan shows a staircase going up to a loft. The loft
needs to be shown on the existing plans.
