I have received and reviewed the plans'for the proposed addition to the above - mentioned
residence. The proposal for the addition has been approved as per plans bearing the

the following conditions:
1. The total number of bedrooms must remain at three without prior approval by this
Department.
2. The area of the existing sewage disposal system and its expansion area must be
maintained.
3. All plumbing fixtures must be updated with water saving devices (i.e. new low
flush toilets, restrictors for shower heads and faucets, etc.).
4. The approval is for the proposed changes only. This approval does not validate
any construction shown as existing that has not obtained proper approvals.
Any other permits or variances required are the responsibility of the applicant and the
jurisdiction of the Town of Putnam Valley.

I have received and reviewed the plans for the proposed addition at the above mentioned
residence. Based on the information submitted, the above mentioned addition cannot be
approved for the following reasons:
1. There is a 4th bedroom and second kitchen on the lower floor.
2. The legal bedroom count for the dwelling is 3. The potential bedroom count of your
proposed addition is 5.
3. The addition of a potential bedroom requires this Department's approval of a revised
septic system plan from a professional engineer.
Please revise the proposed floor plan to reflect no more than 3 potential bedrooms, or have a
professional engineer or registered architect design a sub - surface sewage treatment system
meeting present code requirements.

House Addition /Replacement Guidelines

1) The Putnam County Department of Health must review all proposed additions, which
will result in an increase in living area.
A. Any addition which is considered a potential bedroom requires a formal approval
of plans (Construction Permit) by the Department and plans are to be prepared by
a Professional Engineer or Registered Architect in accordance with applicable
sections of the Putnam County Sanitary Code, unless system is presently designed
for proposed number of bedrooms. , - Plans. wilLpzovide._for the - installation of
- " additional and/or new sewage disposal area meeting rp esent code requirements.
B. The determination of whether a proposed room addition to a house is considered a
bedroom will be made by Department staff based upon:
- location of the room in the house
- size of the room
1.. Accessory rooms such as dens, libraries, studies, computer
rooms, offices, sewing rooms, etc. 'may be considered potential
bedrooms.
2. Large bedrooms, which may easily be divided by a partition
wall, may be considered two potential bedrooms.
3. Storage areas or unfinished portions of the addition may also
be considered potential living area.
C. Any addition which is not a bedroom will require the submission of a plan
prepared by the property owner (to scale) showing the entire house floor plan
existing and proposed.: The determination of what constitutes a potential bedroom
will be made by Department staff (i.e. an office, 8' x 8' may be considered a
potential bedroom). Once the review has been completed the plans will be
stamped noting the number of bedrooms, including potential bedrooms. If the
number of bedrooms remains the same as existing, no further expansion of the
sewage disposal system will be required. If however, it is determined that any
increase in potential bedrooms is proposed then refer to "A" above. A letter from
the Department will be issued indicating total number of existing bedrooms and
no expansion of sewage disposal area will be required and any other permits or
variances required are the jurisdiction of the Town.

2) The Putnam County Department of Health will allow the replacement of an existing
residence utilizing the existing sewage disposal and water supply for the following
reasons:
A. Hardship due to fire or other catastrophic event.
B. Dwelling has become a hazard and risk to human health or safety.
C. Case by case request approved by the Director of Public Health.
The applicant must comply with all of the following:
A. Septic system operating satisfactorily.
B: Potable water supply meets bacteriological standards
C. Square footage of replacement essentially the same as existing structure.
D. Footprint of replacement essentially same as existing structure.
E. Same number of bedrooms as existing.
Note: Definition of what constitutes a bedroom will be made by Department staff
using same criteria in House Addition Guidelines.
F. Approval by local town building and zoning laws.
Note: any increase in square footage of dwelling or increase in number of bedrooms
requires formal submission of plans from licensed engineer or architect meeting present_
code requirements.

Dear Mr. Paravati,
I visited you about two weeks ago regarding attaining BOH approval for finishing the lower level of my house, which was
completed several years ago.
I purchased the house as new construction in October 2000. The house is a 'standard' raised ranch and the lower level is
% above ground (only the rear wall is 80% below grade) and already had rough plumbing for baths and laundry, a finished
sheetrock ceiling and unfinished stud walls around the perimeter and interior unfinished stud walls dividing the space into
4 large rooms. In 2003, after being advised that a Level 3 Sex Offender moved in across the street from me (23 Trail of
Hemlocks), I finished the basement with an apartment for my mother to move in (and be available to watch my son) and a
family room for myself. I did not change anything to the exterior of the house; all doors and windows were preexisting. As
the lower level was already partially completed, and being that it was obviously meant for living space I, regretfully did not
apply for any permits or approvals.
I now wish to sell the house in order to move into a more child friendly neighborhood. However, I understand that I need
BOH approval prior to even applying for a COO. The current COO for my house is for a 3 bedroom residence only. My
septic tank is 1,250 gallons and was installed by the builder in 2000, it is not the original tank from the previous home (see
accompanying invoice from February 2006, from Eastern States Septic, Co). Can you please review the floor plans of the
existing (finished) layout and advise of what modifications I need to make to revert the house back to a 3 bedroom status,
or if the septic is adequate, preferably to a 4 bedroom status.
From my discussion with you, I am hopeful that only minimal modifications will be necessary. I know a portion of the
kitchen (appliances ?) may need to be removed. If it is necessary to eliminate the apartment bedroom, it can be easily
done by removing a 2 door pass through closet between it and my family room and /or opening the bedroom up'to the
living room area of the apartment.
Also enclosed for your reference are the 'basement' plans that were filed with the Town of Putnam Valley by the builder of
my house. Although the outside dimensions are correct, the interior floor plan as completed by the builder was completely
different. The lower level never had a garage (as there was already an existing detached two car garage from the
previous home), the furnace and laundry are actually located in the far right hand corner, and the lower level was roughly
divided into four equal size rooms by unfinished stud walls. In addition to the floor plans, also please find the COO issued
on October 20, 2000 and the building permit issued on May 13, 1999.
Enclosed are pictures of the house with descriptions written on the back for your reference. As you will see the apartment
construction, layout and condition of the premises is excellent, the ceilings throughout the lower level are 9+ feet. If you
are familiar with the neighborhood, you will know that my home is well maintained and the improvements I've made to the
landscaping and my maintenance to the street in front of my house benefits all residents in the area (it is the only portion
of the road that can accommodate 2 lanes and has distance visibility).
Mr. Paravati, I respectfully ask for your help in this matter. Please trust that my wife and I are truly sorry for not going
through the proper channels initially. This was our first home and purchased when we were in our early 20s. We want to
do everything possible to be in compliance with regulations, but naturally do not want to have to destroy the home we
love. I would be more than happy to reimburse you for travel expenses to review the floor plan, or even drive you myself,
to ensure that we do everything necessary correctly to remedy the situation. If I can be of any further assistance you can

Dear Mr. Luke,
Per your review and recommendation during your on -site visit to my home on Monday,
July 10th, I revised the floor plan of my lower level. The remodeling construction should
be completed by the end of this week (- 7/21).
In summary, per your recommendations, the following changes were made:

2) The wall separating the former living/bedroom area was removed.
3) The hallway and closet between the two `halves' of the lower level was removed and
the office size was reduced: both of these changes created a 6' 3" pass /through
opening thus creating one large Family. room.
Can you please review the plan and approve. From my original application on May 19,
2006, you should already have a $100 Money Order, a copy of the survey showing septic
location and a copy of the original COO.
For your reference I also included the original plans submitted on May 19th, on which
you drew lines to show the changes needed. I highlighted your marks in yellow so they
are easier to identify.
Enclosed is a prepaid overnight envelope so you can return the approved plan to me.

ping. Whey they. returned about
four hours later, their home was
ablaze..
Putnam Valley Police received
a call about the fire at approxi-
mately 3 p.m. Police Officer Jo-
seph Dusavage said he was one
of the first people on the scene.
"The whole top of the house
was completely engulfed in
flames," he said. "You couldn't
get inside the house, the flames

parrot, Pan,' also was feared dead
in the fire. A! husky named Tala
had been tied up in the back
yard, but was; found roaming free
when . the Greens arrived. They
also kept chickens and rabbits in

valuable business records.
Charles Green ran a landscaping
business from the house and
Christine Green, owner of the
Metaphysical Book Store in Gar-
rison, kept documents there.
The Greens weren't sure
where' they would spend the
night. A neighbor offered to. put
them up. Charles Green said they
also could stay with his sister in
Cortlandt.
"I'm shaking. I don't know if
it's shock or the cold," Charles
Green said.
