4. Copy of survey showing all well and septic locations on the subject property to the best
of your knowledge. Include date of installation known. Contact this office with any
questions.

I have received and reviewed the plans for the proposed addition to the above- mentioned residence.
The proposal for the addition has been approved as per plans bearing the approval stamp from this
Department dated March 5, 2012. The addition is approved with the following conditions:
1. The total number of bedrooms must remain at three without prior approval by this Department.
2. The area of the existing sewage disposal system and its expansion area must be maintained.
3. All plumbing fixtures must be updated with water saving devices, i.e., new low flush toilets,
restrictors for shower heads and faucets etc.
4. This Department recommends you contact your local Building Department to ensure setbacks
and other current codes can be met.
5. The approval is for the proposed changes only. This approval does not validate any
construction shown as existing that has not obtained proper approvals
Any other permits or variances required are the responsibility of the applicant and the jurisdiction of
the Town of Patterson.

Dear Mr. Garrity:
I have received and reviewed the plans for the proposed addition to the above-mentioned
residence. Based on the information submitted, the above mentioned addition cannot be
approved for the following reasons:
I . The new stair access that leads to the original back bedrooms now partitions the fortner
master bedroom. This former master bedroom is now divided in half by the access
hallway, thereby creating two potential bedroom areas.
2. The legalbedroom count for the dwelling is 3. The potential bedroom count of your
proposed addition is 5.
3. The addition of a potential bedroom requires this department's approval of a revised
septic system plan from a professional engineer.
Please revise the proposed floor plan to reflect no more than 3 potential bedrooms, or have a
professional engineer or. registered architect design a sub-surface sewage treatment system
meeting present code requirements.

The attached plans have been revised in accordance with the comments received from the
Health Department. Specifically, the second floor plan has been revised to show a larger
closet in Bedroom #2, a closet for Bedroom #3 and a bathroom is now shown located
between Bedroom #1 and Bedroom #3.

performed interior alterations. The house plans previously submitted to you show the
house in its current configuration. Putnam Engineering was retained after the
improvements were made, therefore, we do not have sketches of the floor plan as it
existed prior to the alterations.

regarded project has been completed. The following was not submitted with your application:
1. One set of sketches of existing floor plans showing existing conditions only. The plans
must reflect all floors in the house, including the basement, with all rooms noting their
dimensions and use. The plans must also be noted as existing showing owner's name
address and tax map number. Although you do not personally have this information, the
Building Department and/or homeowner may be able to assist. Without this information,
this Department cannot make a determination.
2. Copy of Certificate of Occupancy from the Town of certification from the Building
Department noting the legal bedroom count of the dwelling.
Upon a receipt of a submission, revised to reflect the above comments, this application will be
considered further. Your plans and other supporting documents have been returned for your use.
