GUARANTY OF SEPARATE SEWAGE-SYSTEM
rre'present that I am wholly and completely responsible for the
location, workmanship, material, construction and drainage of the sewage
disposal system serving the above described property, and that it has been
constructed as shown on the approved plan or approved amendment thereto,
and in accordance with the standards, rules and regulations of the Putnam
County Department of Health, and hereby guaranty to the owner, his succes-
sors, heirs or assigns,, to place in good operating condition any part of
said system constructed by me which fails to operate for a period of two
years immediately following the date of initial use of the sewage disposal
system, or any repairs made by me to'such system, except where the failure
to operate properly is. caused by the willful or negligent act of.the occu-
pant of the building utilizing the system.
The undersigned further agrees to accept as conclusive the de-
termination of the Director of the Division of Environmental.Health Ser-
vices of the Putnam County Department of Health as.to whether or not the
failure of the system to operate was caused by the willful or negligent
act of the occupant of the building utilizing the system.

DEC stream protection permit, a letter was sent to
the DEC and I received a reply dated July 26, 1978,

Dear Mr. Haughney:.
I refer to your letter of July.18, 1978 in which you .asked if a permit is
.required for construction-of a foot bridge across a stream on the north side
of:Big Elm Road. The stream identified on your site location map is classi-
fied "D" and as such is not afforded protection under Article 15 of the
Environmental Conservation Law. Therefore, no permit will be required for
alterations to the bed or banks of this stream.

Dear Mr. Kozlowski:
I have reviewed the "as- built" drawings for the sewage
disposal system serving the above- mentioned property.
The Certificate of Compliance was issued in June of 1980 for
a two - bedroom residence. The as builts indicate that 480
lineal feet of fields were installed. Based on an 11 -15
minute perc rate, 250 lineal feet is required for a two
bedroom residence and 375 lineal feet for a three bedroom
residence.
Based on this information, it appears the sewage disposal
system is adequate for a three bedroom residence.
Please..submit.a drawing of your floor plan, as it exists now,
and a drawing of the proposed addition, for our review and
approval.
If you have any questions concerning this matter, please

system design. We are currently considering remodeling our home.
Presently, we have two bedrooms and one bath. We are considering adding one
bedroom and one bath to the existing house.
Please let us know if any improvements to the system may be necessary and
what procedures we must follow to get BOH approval for this project.

% have received and revieved the plans for the proposed addition on the above -
mentioned residence.
The plans indicate that three bedrooms and a bath are to be constructed on the
proposed second floor.
Existing bedrooms, designated as 1 and 2, will be relocated to the proposed
second floor and the area previously occupied by bedrooms 1 and 2 dill be
converted into the proposed living room.
Review of the files pertaining to the original construction of the subsurface
sewage disposal system for the above captioned lot indicates a percolation rate
of 12 ruin /inch and 415 feet of absorption trench were constructed. Current code
requirements would dictate the minimum of 375 feet of absorption trench be
constructed for a three bedre!bp house.::bes .db__ tpiie. �n�i ]._�a4.a -�vai Table.
Therefore, based on the information it appears the sewage disposal system is
adequate for a three bedroom house.
The survey indicates that sufficient area exists to expand or repair the sewage
disposal system, should it become necessary in the future. Therefore, based on
the information submitted, the above mentioned addition is approved with the
following conditions:
1. The total number of bedrooms must remain at three nithout prior approval by
this Department.
2. The area of the existing sewage disposal system, and its expansion area, must
be maintained.
3. All plumbing fixtures must be replaced or updated with rater saving devices,
i.e., low flush toilets, restrictors for shower heads and faucets, etc.

As per our conversation this morning, enclosed please find the
items necessary to obtain Board of Health approval for our proposed
additions--
1. Existing floor plan
2. Proposed floor plan and extension
3. Survey with well and septic information
4. Copy of letter from Bill Hedges of the Putnam C. Dept. Health
dated 3/88.
As we had discussed,'we are scheduled to appear at the Zoning Board
of Appeals meeting on 9/19/90. Until yesterday we did not know that
Board of Health approval was needed for this meeting and we had
thought that the above mentioned letter in item #4 was all that was
necessary.
I understand that it may take -up to two weeks to hear from your
department. Would it.be possible to obtain approval. by Sept. 19th?
If se, we would be extemely grateful and would personally pick up
at your office the necessvr documents required for the ZBA meeting.

I have received.and reviewed the plans for the proposed addition on the above -
mentioned residence.
The plans indicate that three bedrooms and a bath are to be constructed on the
proposed second floor.
Existing,bedrooms, designated as 1 and 2, will be relocated to the proposed
second floor and the area previously occupied by bedrooms i and 2 will be
converted into the proposed living room.
Review of the files pertaining to the original construction of the subsurface
sewage disposal system for the above captioned lot indicates a percolation rate
of 12 min /inch and 415 feet of absorption trench were constructed. Current code
requirements would dictate the minimum of 375 feet of absorption trench be
constructed for a three bedroom house based on the.soil data available.
Therefore, based on the information it appears the sewage disposal system is
adequate for a three bedroom house.
The survey indicates that sufficient area exists to expand or repair the sewage
disposal system, should it become necessary in the future. Therefore, based on
the information submitted, the above mentioned addition is approved with the
following conditions:
1. The total number of bedrooms must remain at three without prior approval by
this Department.
2. The area of the existing sewage disposal system, and its expansion area, must
be maintained.'
3. All plumbing fixtures must be replaced or updated with water saving devices,
i.e., low flush toilets, restrictors for shower heads and faucets, etc.

Dear Mr. Kozlowski:
I have reviewed the "as- built" drawings for the sewage
disposal.system serving the above - mentioned property.
The Certificate of Compliance was issued in June of 1980 for
a two_ bedroom residence. The as builts indicate that 480
lineal feet of fields were installed. Based on an 11 -15
minute perc rate, 250 lineal feet is required for a two
bedroom residence and 375 lineal feet for a three bedroom
. •.... residence. .. . _ ... _ . ....... . .
Based on this information, it appears the sewage disposal
system is adequate for a three bedroom residence.
Please submit a drawing of your floor plan, as it exists now,
and a drawing of the proposed addition, for our review and
approval.

dated 3/88.
As we had discussed, we are scheduled to appear at the Zoning Board
of Appeals meeting on 9/19/90. Until yesterday we did not know that
Board of Health approval was needed for this meeting and we had
thought that the above mentioned letter in item #4 was all that was
necessary.
I understand that it may take up to two weeks to hear from your
department. Would it be possible - t® obtain --apprsval�,hy Sept. - 19th?
