2. Submission of as built repair sketch in duplicate showing:
a. owner's name.
b. Site Street Name, Town and Tax Map number.
c. Location of installed components tied to two fixed points (e.g.,house corners).
d. System description (e.g., 1250 gal. concrete septic tank, three precast 6' diaim. x 6' deep
drywells surrounded by one foot + gravel).
e. Installer's name and number.
3. System repair to be performed in accordance with the above proposal and conditions.

I have received and reviewed the floor plan of your existing residence located on
Fair Street in the Town of Patterson.
The floor plan indicates three existing bedrooms upstairs and a basement area
consisting of a garage, family room, laundry room, bathroom and bedroom.
Apparently the basement area was renovated without a permit or Certificate of
Occupancy issued by the Town of Patterson.
Records indicate that the basement area was renovated prior to 1984.
The residence is located on eight acres and is served by an individual well and
sewage disposal system. The sewage disposal system was reconstructed in 1990.
Based on the information submitted, this Department has no objection to the Town
of Patterson issuing a Certificate of Occupancy for the above mentioned
renovation.
Acceptance by this Department is for the individual water supply and sewage
disposal area only.
Any other permits or variances required are the responsibility of the applicant
and the jurisdiction of the Town of Patterson.
