Public Health Director Re: Suspension of Construction Permit #P -25 -89 Seelke, Quebec Road (T) Patterson TM #51 -7 -1, 2 & 3 Dear Mr. Shilling: I have receive_ and reviewed your letter of July 9, 1991 concerning_,the suspension e a ove mentioned construction permit. The application for a construction permit on TM #51 -7 -1, 2 and 3 was submitted by John Prentiss, P. E. for our review on May 15, 1989. This application was reviewed as a new submission and not a renewal of an existing permit. There is no record of a previous 1979 approval, as your letter indicates. Mr. Seelke did apply for and received approval of lots i4689 -4701 (Tax Map #52 -5 -9 later changed to Tax Map #52- 5 -9.1), also located on Quebec Road in 1979. This is a different parcel of land, located on the east side of Quebec Road. The letter of January 29, 1991 by John Karell of this Department explains the reasons for the suspension of the construction permit #P- 25 -89, TM #51 -7 -1, 2 & 3 and also outlines the requirements of this Department for the reinstatement of _ the permit. The proposed sewage disposal system on Seelke's parcel #51 -7 -1, 2 and 3 Permit #P25 -89, appears to be in direct line to drainage of the individual water supply on the parcel to the south. (P -1 -88, J. V. Construction, Quebec Road, TM #51 -7 -4 & 5). i— minimum of 200:; separation is required. 100' is shown. 2. The topography shown on the plans for this property does not appear to accurately reflect site conditions. .Therefore the following information must be submitted for our review prior to considering reinstatement of this construction permit ( #P- 25 -89). 1. The plans must contain topography at two foot intervals, prepared by a licensed surveyor in order that we may determine direct line of drainage. 3. The application for a construction permit on TM #51 -7 -1, 2 and 3 was submitted by John Prentiss, P. E. for our review on May 15, 1989. This application was reviewed as a new submission and not a renewal of an existing permit. There is no record of a previous 1979 approval, as your letter indicates. _ Mr. Seelke did apply for and received approval of lots #4689 -4701 (Tax Map #52 -5 -9 later changed to Tax Map #52- 5 -9.1), also located on Quebec Road in 1979. This is a different parcel of land, located on the east side of Quebec Road.
