Dear Mr. Shilling:
I have receive_ and reviewed your letter of July 9, 1991 concerning_,the
suspension e a ove mentioned construction permit.
The application for a construction permit on TM #51 -7 -1, 2 and 3 was submitted by
John Prentiss, P. E. for our review on May 15, 1989.
This application was reviewed as a new submission and not a renewal of an
existing permit. There is no record of a previous 1979 approval, as your letter
indicates.
Mr. Seelke did apply for and received approval of lots i4689 -4701
(Tax Map #52 -5 -9 later changed to Tax Map #52- 5 -9.1), also located on Quebec Road
in 1979.
This is a different parcel of land, located on the east side of Quebec Road.
The letter of January 29, 1991 by John Karell of this Department explains the
reasons for the suspension of the construction permit #P- 25 -89, TM #51 -7 -1, 2 & 3
and also outlines the requirements of this Department for the reinstatement of _
the permit.
To date, no revisions have been submitted to this Department for our review.

Please be advised that the permit to construct a sewage disposal system and individual
water supply on the above mentioned parcel is hereby, ussend_ed, effective January 29, 1991,
for the following reasons:
1. The proposed sewage disposal system on Seelke's parcel #51 -7 -1, 2 and 3 Permit #P25 -89,
appears to be in direct line to drainage of the individual water supply on the parcel to
the south. (P -1 -88, J. V. Construction, Quebec Road, TM #51 -7 -4 & 5). i— minimum of 200:;
separation is required. 100' is shown.
2. The topography shown on the plans for this property does not appear to accurately
reflect site conditions.
.Therefore the following information must be submitted for our review prior to considering
reinstatement of this construction permit ( #P- 25 -89).
1. The new well on the lot to the south TM #51 -7 -4 & 5 must be accurately located.
2. The plans must contain topography at two foot intervals, prepared by a licensed
surveyor in order that we may determine direct line of drainage.
3. If necessary, the sewage,disposal system must be redesigned to respect all minimum
separation distances as stated in our rules and `regulations.

This permit to construct one water well as set forth above is granted under the
provisions of Subpart 5 -2 of Part 5 of the New York State Sanitary Code, and
provided that within thirty (30) days of the completion of water well construction,
the applicant shall:
1. Pump the well until the water is clear.
2. Disinfect the well in accordance with the requirements of the Putnam
County Health Department attached to this permit.
3. Submit a Well Completion Report on a form provided by the Putnam County
Health Department.

This permit to construct one water well as set forth above is granted under the
provisions of Subpart 5 -2 of Part 5 of the New York State Sanitary Code, and
provided that within thirty (30) days of the completion of water well construction,
the applicant s.hall:
1. Pump the well until the water is clear.
2. Disinfect the well in accordance with the requirements of the Putnam
County Health Department attached to this permit.
3. Submit a Well Completion Report on a form provided by the Putnam County
Health Department.

Dear Mr. and Mrs. Seelke:
This in response to your letter which I received on August 21,
1992. I am surprised at the contents of same in that I have
advised you on more than one occasion that the remaining work to
obtain Board of Health approval must come from your engineer.
When you retained this office, you indicated that the lot in
question had Board of Health approval and that the Board of
Health somehow acted illegally in not recognizing that approval.
At a meeting I attended with your engineer and with William
;;GUges from the Boa- - __of. Health it became readily apparent that
your lot never received Board of Health Approval and that your
neighbor's building on the adjoining lot made Board of Health
approval more difficult. Mr. Hedges advised that in order to
obtain Board of Health' approval for your lot, he required a
topographical study and perhaps a redesign insuring that no wells
would be contaminated by your septic. A topographical map was
prepared and forwarded to Mr. Prentice and the Board of Health.
I must emphasize to you now that the revised septic must come
from your engineer and be presented to the Board of Health.
In conclusion, I feel that though this office did everything
possible to clarify the issues and make Board of Health approval
accessible to you. Your engineer must submit the necessary data.
I would strong suggest that you push your current engineer to
complete this work.
If I can be of further assistance to you, please do not hesitate
to contact me.

Please be advised that the permit to construct a sewage disposal system and individual
water supply on the above mentioned parcel is hereby suspended, effective January 29, 1991
for the following reasons:
1. The proposed sewage disposal system on Seelke's parcel #51-7 -1, 2 and 3 Permit #P25 -89,
appears to be in direct line to drainage of the individual water supply on the parcel t
the south. (P -1-88, J. V. Construction, Quebec Road, TM #51 -7 -4 & 5). A minimum of 20C
separation is required. 100' is shown.
2. The topography shown on the plans for this propert;, does not appear to accurately
reflect site cnnditions.
Therefore the following information must be submitted for our review prior to considering
reinstatement of this construction permit (*P- 25 -89).
1. The new well on the lot to the south TM #51 -7 -4 & 5. must be accurately located.
2 The plans must contain topography at two foot intervals, prepared by a licensed

I have received and reviewed your letter of July 9, 1991 concerning the
suspension of the above mentioned construction permit.
The application for a construction permit on TM #51 -7 -1, 2 and 3 was submitted by
John Prentiss, P. E. for our review on May 15, 1989.
This application was reviewed as a new submission and not a renewal of an
existing permit. There is no record of a previous 1979 approval, as your letter
indicates. _
Mr. Seelke did apply for and received approval of lots #4689 -4701
(Tax Map #52 -5 -9 later changed to Tax Map #52- 5 -9.1), also located on Quebec Road
in 1979.
This is a different parcel of land, located on the east side of Quebec Road.
The letter of January 29, 1991 by John Karell of this Department explains the
reasons for the suspension of the construction permit #P- 25 -89, TM #51 -7 -1, 2 & 3
and also outlines the requirements of this Department for the reinstatement of
the permit.
To date, no revisions have been submitted to this Department for our review.
